Severity: Moderate
Adverse Effects:
Bleeding, injection-site hematoma, rare HIT (much lower risk than UFH)
Contraindications:
Active major bleeding, history of HIT, severe renal impairment (CrCl <30) unless dose adjusted
Precautions:
Adjust dose in renal impairment; monitor anti-Xa levels in pregnancy/renal failure; partially reversible with protamine
