Severity: Severe
Adverse Effects:
Diarrhoea, rash, neutropenia, infusion reactions; potential cardiotoxicity (esp. with trastuzumab); embryo-fetal toxicity
Contraindications:
Severe heart failure; pregnancy
Precautions:
Baseline & periodic LVEF monitoring; effective contraception during and after therapy
